Pregnancy Care Management is a program for Medicaid eligible pregnant women who have specific priority risk factors that may affect their pregnancy. Referrals are made directly by OBGYN doctors, family practices, or local support agencies. The goal of the program is to encourage a healthy birth outcome for mom and baby and to also reduce the risks of preterm labor statewide.
